Comprehensive Primary Care Plus (CPC+) is an advanced primary care medical home model that aims to strengthen primary care through regionally based multi-payer payment reform and care delivery transformation. CPC+ includes two primary care practice tracks with incrementally advanced care delivery requirements and payment options to meet the diverse needs of primary care practices.
Partnerships under this program give practices additional financial resources and flexibility to make investments, improve quality of care, and reduce the number of unnecessary services their patients receive.

CPC+, is unlike any other method of medical billing because it was designed to reward and incentivize primary care physicians for the service they provide to all their patients, not just the physician’s Medicare patients. Clinicians who are enrolled in the CPC+ program are eligible to receive bonus payments on their Medicare charges.
